The job is to take the lead on their existing and future projects to reduce social isolation, and to develop and support the referral and befriending scheme. The post-holder will deliver an inclusive service to reduce social isolation for older people, working closely with the project worker. They will develop the befriending scheme, and co-ordinate and undertake home visit assessments to older people in the community.
